They may be saving the Canadian import in case there's yet another strike. But that means there's no air date for when it might actually be seen down south. It could be next season - or it could be later than that. Will viewers still remember it by then?

Transplant must have done better when recorded viewing and Peacock were added in since it was very average to poor in the ratings on NBC. Here of course the show did well for CTV.
I guess since NBC isn't exactly setting the world on fire in prime time, Transplant is a good familiar show to have in the back pocket, and cheap to run. Also some new dramas could get well under 2 million viewers which is not out of the question for US prime time scripted shows.

Very tough, though, to have the storyline continue if it's going to be interrupted by months, if not more than year of delays before it gets shown. Will anyone remember where they left off when it finally comes back? And will viewers still care? That's the danger of such a long layoff.

Not really, with the first 3 seasons are still available on Peacock, it is easy to refresh where they left off or pick up new fans. If the viewers didn't care NBC wouldn't have brought Transplant back the first time after the long break. Obviously a long gap is not a big deal if the executives seem to be committing to the last season at some point.
They are bringing it back (eventually) since the show likely was more popular than the ratings showed, it is familiar to the audience and most of all CHEAP for NBC to run. A good show to have when the competition is into repeats.
