Canada's largest newspaper has a hard and fast rule: they refuse to publish any pictures that may have been created by Artificial Intelligence. So how did one that was A.I. generated make it into the paper? 

The answer is a convoluted tale of mistakes and an initial error that originated with a service that provides them with generic images, and slipped through their screening process.

The accident is considered so serious by the Star that it is considering parting ways with the provider.
